Photo /Video/Audio Recording Consent and Release Notice

I, on behalf of myself and any other adult Subject(s), and, if applicable, on behalf of any minor child for whom I am the parent, legal guardian, or an adult duly authorized by the parent or legal guardian, acknowledge that by purchasing or accepting tickets (for myself or for others), attending or participating in any event or class hosted or facilitated by Funky Dog Theatrical Inc., a.k.a. Funky Dog Improv, 587 W. Eau Gallie Blvd #101, Melbourne, FL 32935 (whether on-premises or off-premises), or by contracting with Funky Dog Theatrical Inc., a.k.a. Funky Dog Improv in any way, I hereby give consent to Funky Dog Theatrical Inc., a.k.a. Funky Dog Improv:

a.) to record my likeness and/or the likeness of the Subject(s), and my voice and/or the voice of the Subject(s), on video, audio, photographic, digital, electronic, or any other medium, and to use my name and/or the name of the Subject(s) in connection with these recordings; and

b.)  to use, reproduce, exhibit, or distribute these recordings, in whole or in part, in perpetuity and throughout the universe, in any and all media now known or later developed (including but not limited to print publications, videotapes, home video, CD-ROM, internet, and other electronic/digital media) for any purpose Funky Dog Theatrical Inc., a.k.a. Funky Dog Improv, and those acting under its authority deem appropriate, including promotional, recruiting, advertising, commercial, and non-commercial use.

I understand that no compensation shall be given to, and no compensation shall be received by, myself or the above-mentioned Subject(s) for such use, and I waive any right to inspect or approve the finished product or its use. This consent and release is irrevocable, worldwide, and perpetual.

I hereby release Funky Dog Theatrical Inc., a.k.a. Funky Dog Improv, from any and all claims, demands, actions, suits, liabilities, or damages whatsoever arising out of or in connection with the use of my likeness, voice, or name, or that of the above-mentioned Subject(s), in any medium. Each adult Subject attending under a ticket purchased or accepted on their behalf is deemed to have accepted these terms by accepting or using the ticket.

If the Subject is a minor child, I represent that I am the parent or legal guardian of the Subject, or that I have obtained the consent and authority of the Subject’s parent or legal guardian to execute this release on their behalf. I understand and agree that Funky Dog Theatrical Inc., a.k.a. Funky Dog Improv, cannot verify or control who purchases tickets on behalf of a minor, and that it is the sole responsibility of the ticket purchaser to ensure they have the proper authority or consent. This release shall be binding upon the Subject, their heirs, legal representatives, and assigns.
